One of the requirements for this merit badge is to have a physical and dental exam and to explain the importance of both of these exams toward your personal fitness.
You have all completed the physical exam. I need proof that you have completed a dental exam. Please email me a picture of a dental exam bill to prove that you've had an exam in the last 6 months (or in the future).
To explain the importance of these exams, fill out this form. Please use the Personal Fitness pamphlet to help you with the answers. Do not simply make-up answers to the questions. Also, be sure to hit the submit button after you have answered all of the questions!
